Generally payment is expected at the time of service. We accept cash or checks.

Patents with traditional (indemnity) insurance are responsible for the co-payment/deductible/co-insurance at the time of service. We will ask you to sign an authorization form to enable us to seek payment from your insurance company directly to us.

However, if your insurance has not paid your balance in 30 days this balance will become your responsibility. At present, we accept Medicare, Medicaid, Tricare, Blue Cross Blue Shield, Aetna, CareFirst, Anthem, Anthem HealthKeepers, Priority Health Care, Peninsula Health Care, National Capital PPO, First Health, CCN Network, Humana/ChoiceCare Network, Cigna Healthcare, MAMSI, Aliance PPO, Multiplan PPO Network, Three Rivers Provider Network, CitizensHealth, Value Card, PHCS, UnitedHealthcare and Great-West. Please call us to determine if your plan has out of network benefits that will cover services at Pain Medicine & Rehab.

Patients with Workers' Compensation or Automobile Insurance will be asked to complete authorization forms and payment will be seeked from their employers or / and insurer. If payment is refused in a Workers' Compensation case, we will submit an itemized bill to the Workers' Compensation Commission. If you have a lawyer that is helping you with your injury, we will work with him to arrange for payment of your medical bills.

Anti-Aging Medicine services and medications are not covered by insurance. Please call us for rates.Weight Loss Management services and medications are covered by most comercial indemnity insurance. However, Medicare and Medicaid do not cover these services
